Home Care Program
Support and Independence for Elderly and Disabled Adults
Partnership’s Home Care Program assists elderly individuals and disabled adults in maintaining their independence within the comfort of their own homes. Our Case Managers assess each client’s needs, develop personalized care plans, and assign dedicated Care Specialists to provide services such as housekeeping, grocery shopping, and non-medical personal assistance.
How to Apply?
Referrals are accepted exclusively through the TN Department of Human Services – Adult Protective Services.
Who Can Apply?
- Identified by Adult Protective Services as needing assistance with daily living activities
- Seniors (60+) requiring support to maintain independence
- Disabled adults (18-59) requiring support
