Elder Support Services
Many of our senior neighbors need a little extra help to remain independent. Without family support, where can they turn? Partnership's Elder Support Service programs offer personal attention to individuals over 60 years of age and disabled adults ages 18-59 with the mission of securing a safe, independent way of living.
Partnership’s Elder Support Services Program is designed to provide support to individuals who require assistance with activities of daily living in order to maintain their independence. In addition, advocates are available for individuals who reside in long term health care facilities. The Elder Services staff can provide assistance with health services, home-based services, public benefits and advocacy services. The staff is committed to promoting independence and preserving the dignity of elders and people with disabilities.

Community Case Management
Case Managers help elderly and disabled individuals maintain greater independence in the comfort of their own homes.
Home Care Program
Partnership's Home Care Program assists individuals over 60 years of age and disabled adults 18-59 to remain self-sufficient by assisting with the daily tasks of living, such as light housework, grocery shopping, and accompaniment to medical appointments.
Long Term Care Ombudsman
Long Term Care Ombudsman provides advocacy for residents in nursing homes and assisted living facilities. Advocates work to prevent elder abuse and ensure the needs of residents in assisted living facilities are properly attended to.
